Quality and Outcomes Framework (QOF)

The Quality and Outcomes Framework (QOF) is a voluntary annual reward and incentive programme for all GP surgeries in England

QOF contains four main domains which include areas relating to Clinical, Organisational, Additional Services and Patient Experience. Each domain consists of a set of achievement measures, known as indicators.

2012/13 QOF measured achievement against 140 indicators; practices score points on the basis of achievement against each indicator, up to a maximum of 1000 points.

An important feature of QOF is the establishment of disease registers. While it is recognised that these may not be 100% accurate, it is the responsibility of the practice to demonstrate that it has systems in place to maintain a high quality register.

The QOF gives an indication of the overall achievement of a surgery through the points system. Practices aim to deliver high quality care across a range of areas for which they score points. Therefore the higher the score, the higher the financial reward for the practice.
