Practices are asked to please ensure they fully read the content of the letter and act accordingly in good time for completion of the survey.
The survey collection will open on Monday 11 April 2011, and practices have until Thursday 5 May to complete their submission to ImmForm. This is a mandatory collection for data in relation to all patients aged 65 and over, those who have had a pneumococcal vaccination in the 2010-11 year, and those who have had a pneumococcal vaccination ‘ever recorded’. This is the minimum data that practices must submit to ImmForm by the closing date of 5 May.
